Every potential member is required to fill out this form below before they can enroll.
Medical form
It is important the club is kept informed of medical conditions & history so that coaches are aware, just in case anything happens during swimming. Please use this form to update us, if necessary.
SASA form
Dyce ASC is affiliated to the Scottish Amateur Swimming Association. All swimmers, coaches, volunteers and committee members must become members of SASA to take part in Dyce ASC activities. The form attached must be returned to the Membership Secretary with appropriate fee (please check current fee prior to sending as a pro rata fee may be applicable).
